2008 CHW Community Grants
The Catholic Healthcare West Greater Sacramento Service Area is very pleased to announce the availability of applications for the 2008 funding cycle of the Catholic Healthcare West Community Grants Program.
The intent and goal of the CHW Community Grants Program is to encourage and enable CHW hospitals to reach out and partner with neighborhood non-profit organizations that also are striving to improve the health status and quality of life of the residents in our communities. Responding to needs identified in our 2008 Community Needs Assessment, our focus will be on funding programs or projects that address access to care or disease prevention. Grant requests may be made for any amount up to a maximum of $25,000.
Specifically, the criteria for this funding cycle will be based on the following priority areas:
1) programs or services that assist people in overcoming the barriers that frequently inhibit their access to primary health service (medical, dental and behavioral), or
2) prevention and treatment of chronic health issues (diabetes, cancer, asthma, obesity)

to learn more about the CHW Community Grants Program. In order to manage the large number of inquiries we receive each year, we first ask for a Letter of Intent to be submitted in this format that briefly describes the project and the applicant agency.
In order to be considered, a completed copy of the enclosed Letter of Intent should be submitted via e-mail to

. Please use the above email address for all grant-related e-mail communication. A local committee will review the Letters of Intent and request full proposals from those agencies whose letters outline projects that best meet our program’s goals for this year.
